[Cialug] Simple ticketing system

I use Spiceworks as my To-Do list, but it has all these features. Cloud or self-hosted, but closed-source.

I've heard mixed things about osTicket, but it has an open source version.

We've been using Freshdesk for years, and it does everything we need
(which includes all of the requirements you stated). If you're not
interested in self-hosting, this may be a good option. We are on their
"Sprout" plan, which is their entry-level free plan.
