Here's a weird one:

Have an external IP listed in access.db:

access:68.89.154.121    OK
Binary file access.db matches

The IP does not show up in any RBLs [per mxtoolbox], ..

An email from the IP is rejected by Amavis:

Any thought on why Amavis would reject the email even thought it is
whitelisted?
