So you repaired permissions and restarted? Have you had any other  
problems with your system lately? Apps quitting, freezing, other  
anomalies?

You could try fixamacsoftware.com's Printer Setup Repair. Make sure  
you get the right one for your OS X version. It's got a lot of repair  
options, just poke around. One thing that confuses most people is  
that each icon is a button.
