[DM-MUG] OS X stqartup screen while running OS X?

Here is a real weird one I have never seen before. My wife's iBook 
runs all the time. A few minutes ago she told me that she was running 
without OS X because the startup window was showing. Since she had 
been actively using it I was puzzled but when I look the box with the 
Apple logo, "Starting OS X" and the blue bar clear across it is 
showing even though OS X is running. It looks like it started up 
without bothering to power down.  I did a restart in safe mode and 
run fsck -fy. It ran through the expected routines and made some 
repairs, then ran through them again!! It reported that the system 
was modified but would not return to the prompt so I finally powered 
down and then back up. I'm doing a backup then will reboot in safe 
mode and try Applejack.

Any insight would be helpful.
