I presume you're using Panther or at least Jaguar.
Are you using the three-RCA cable that plugs into the A/V port? I've never
been able to get that to work on mine either.
I use a cable that plugs into the mini-VGA port (next to the A/V port) and
terminates in an RCA and S-Video output. The cable is listed on Apple's
site:

Using this RCA/S-Video cable, and a split-stereo RCA cable in the A/V port,
check the following:
Go to your Displays preferences and check the box to show Displays on the
menubar.
>From the Displays icon in the menubar, Detect Displays.
Give your iBook a moment to detect the VGA/VCR/TV setup, then go to the
Displays icon again.
You should show (1) your iBook's display at your preferred resolutioh and
(2) the VGA display (actually your TV/VCR setup) with NTSC and PAL options.
Select a resolution that matches your iBook display, and NTSC.
