I've been using php-cgi.exe with IIS for quite a while, with both PHP4 and
PHP5, and it seems to work just fine.

Jon,
The isapi dll is a little flaky at times. Try changing it to use php-cgi.exe
or php-win.exe
The access violation errors may be permissions to the sessiondata and/or the
uploaddata folders. Set them to allow IUSR_ modify permissions.
