Unbound meets these requirements.  I've only used it a little bit
though as I prefer dnscache.  The default config file has e.g.

 # interface: 192.0.2.153
 # interface: 2001:DB8::5

 # outgoing-interface: 192.0.2.153
 # outgoing-interface: 2001:DB8::6

 # access-control: 127.0.0.0/8 allow
 # access-control: ::1 allow

and what it calls stub and forward zones to pass queries for specific
domains to specified authoritative or recursive servers.
