I have it set up for about 30 hosts and have send
page notifications to a tmobile sidekick.

Escalations are set up to page and email additional
people if the problem doesn't get fixed.
I haven't had a problem sending pages to US Cellular,
TMobile, or Verizon.

The docs are lengthy but pretty detailed.
I recommend setting up one or two hosts and
a few service checks on them.  Once you get
that done its a piece of cake to add more.
It's not that difficult its just time consuming.

BTW, someone asked in a meeting a while back if you could
limit what hosts and services a user could view.
It's default behavior that you can only view checks for
which you are listed as a contact.  I've always needed
to view everything so during set up I add myself to the
'authorized_for_all_services' and 'authorized_for_all_hosts'
in the cgi.cfg.
