I have had very good luck with gocr for the times that I have needed
ocr.  http://jocr.sourceforge.net/

For the PDF output piece, I have found PDF::API2 to be excellent, if
you grok perl.  http://search.cpan.org/~areibens/PDF-API2-0.41/
